Endocan (also known as endothelial cell-specific molecule-1 [ESM-1]) is an endogenous proteoglycan secreted by vascular endothelial cells which plays an important role in angiogenesis. 8: ObjectiveThe aim of this study was to investigate serum levels of endocan in BD patients in comparison with healthy controls and correlation with disease activity scores. 9: MethodsSerum levels of endocan were measured by ELISA in BD patients (n = 72) with different clinical manifestations including ocular-BD (n = 20), neuro-BD (n = 12), vascular-BD (n = 10), GIT-BD (n = 10), cardiac-BD (n = 8) as well as those without any organ involvement (n = 12). Healthy controls consisted of age- and sex-matched healthy volunteers (n = 32). Disease activity was assessed using Behçet’s Disease Current Activity Form (BDCAF) score. 10: ResultsSerum levels of endocan were significantly higher in BD patients compared with healthy controls [median (IQR): 1.88 ng/mL(1.29–4.23) vs 1.18 ng/mL(0.85–1.78), p < 0.001]. When BD patients were divided into two groups according to median value of serum endocan levels (1.88 ng/mL), those with serum endocan levels above median had higher BDCAF scores [median (IQR): 5(4–9) vs 1(0–4), p < 0.001]. When we divided BD patients into two groups according to current organ involvement status as “current organ involvement” group included patients who had current organ involvement at the time of study whereas “no current organ involvement” group included those without any organ involvement at that time point [e.g., patients had neurological involvement at diagnosis but currently did not have any neurological manifestations], there was no statistically significant difference between two groups regarding serum endocan levels [median (IQR): 2.41 ng/mL(1.40–4.28) vs 1.48 ng/mL(1.22–4.33), p = 0.880]. Serum endocan levels did not correlate with Behçet’s Disease Current Activity Form scores in each subgroup analysis. 11: ConclusionWe found that serum levels of endocan were significantly higher in BD patients compared with healthy controls suggesting that endocan might be involved in pathogenesis of BD. 12: ## Introduction 13: Behçet’s disease (BD) is an autoimmune disorder characterized by systemic vasculitis that involves multiple organ systems including skin [erythema nodosum-like lesions or aphthous ulcers], joints [arthritis or arthralgia], gastrointestinal tract [GIT] [inflammation or ulceration], eyes [uveitis], nervous system [neuro-BD] [inflammation of cranial nerves or meninges], heart [cardio-BD] [myocarditis or pericarditis] and vessels [vascular-BD] [inflammation or thrombosis] [1]. It is more common among populations living along Silk Road from Japan to Mediterranean countries especially Turkey where it has highest prevalence rate compared with any other country around the world [2]. The exact pathophysiology underlying BD remains unclear but genetic predisposition along with environmental factors such as streptococcus sanguinis infection seem to play an important role in its development [3]. 14: Endocan also known as endothelial cell-specific molecule-1 (ESM-1) is an endogenous proteoglycan secreted by vascular endothelial cells which plays an important role in angiogenesis as well as inflammation process through binding with integrins αvβ3/αvβ5 expressed on monocytes/macrophages promoting their migration into inflamed tissue thereby enhancing local immune response [4]. Serum levels of ESM-1 have been reported elevated in various inflammatory diseases including rheumatoid arthritis RA), systemic lupus erythematosus SLE) Crohn’s disease CD), ulcerative colitis UC), psoriasis PsO) suggesting its potential use as a biomarker for monitoring disease activity as well as response towards treatment options available today [5]. 15: The aim of this study was firstly conducted research investigating serum levels of endocan in BD patients compared with healthy controls secondly correlation between serum levelsof endocanand disease activity scores. 16: ## Material and methods 17: ### Study population 18: This study included consecutive adult patients diagnosed with BD according to International Study Group Criteria for Behçet’s Disease at our tertiary referral center between January–June 2021 were enrolled into this study after providing written informed consent approved by local institutional review board/Ethics Committee before beginning data collection process which took place over period mentioned above timeframe described earlier hereinafter referred simply ‘study period’.
